Department of Transportation Violation
SAP-RTD-Substance Abuse Professional Return to Duty
Have you been notified by your employer that you have a violation?
Positive Pre-Employment Drug or Alcohol Screening
Positive Random Drug or Alcohol Screening
Positive Post Accident Drug or Alcohol Screening
Positive Reasonable Suspicion Drug or Alcohol Screening
Positive Drug or Alcohol Screening that qualifies as a DOT Violation and requires that you see a SAP
I provide what is required under DOT-SAP-RTD process which includes:
Initial evaluation
Recommendation
Follow-up
Follow-up testing plan
Initial evaluation-this is an in person face to face meeting which will take approximately 2hours of your time during which I will conduct a "clinical interview" I will be collecting information:
you will be contacted by me within 24 hours with instructions for what action you will need to take, examples being: 8 hours of counseling to address the impact of drug or alcohol abuse, or 10 hours of education that teach about the impact of drug or alcohol abuse.
I will never be able to answer how long you will be ineligible to work until the initial evaluation has been completed.
I understand that you will be needing to get back to work expeditiously-keep in mind getting you back on the road is my initiative-public safety my first priority.
once you have completed whatever my recommendation is then you will contact me for the Follow-up interview, you are required to meet with me and present proof that you complied with initial recommendation - during that meeting compliance must be determined.
you will be provided with a Follow-up letter which will state that you can be considered for Return to Duty pending a negative alcohol and/or drug screening
there will be additional Follow-up alcohol and/or drug testing as required, however you will not be notified-this information will be forwarded to the employer where they will then have responsibility of following testing plan-if you are not employed -once you are employed I will forward this information to the DER-designated employer representative.

- date of violation
- employer information
- your information-name, ss#, address, contact number
- history of previous violation
- history of drug or alcohol abuse
- history of any counseling or treatment
